Registration forms are available as a PDF download. Please read the following instructions carefully!
  • Pages 1-8 must be filled out and signed and returned in person with registration money to the office. Please do not mail in your registration packets.
  • Page 9 is after care. Please fill out and sign if needed.
  • Page 10 is request for transfer for records for new students ONLY.
